To request the use of Mancos School District facilities, please download the form in Word format; or download a PDF version. A signature on the form signifies receipt and acceptance of the following policy, regulation, and exhibit.
If you are planning an event involving food, you will also need to submit a Vendor Application for Temporary Food Events to the Montezuma County Public Health Department at 106 W North St, Cortez, CO 81321. Please click here for the form.
