If you are planning an event involving food, you will also need to submit a Vendor Application for Temporary Food Events to the Montezuma County Public Health Department at 106 W North St, Cortez, CO 81321. Please click here for the form.
PLEASE NOTE: Forms that are dropped off, faxed, or mailed to the district must be signed by the requestor. The address is: Mancos School District Re-6, 355 W Grand Ave, Mancos CO 81321. Fax to: 970-533-7537. A form sent via e-mail will be considered signed by the requestor.
Volunteer Application By volunteering, you can help give time back to the staff so that they can do what they have been hired to do - educate our students! In addition to several regularly scheduled positions, many of our special programs and projects wouldn’t happen without the help of volunteers, so thank you for considering spending some of your valuable time helping to improve our schools and our Community!
