The Mancos School District Athletic forms have been updated with new forms mandated by the school district and CHSAA. These forms need to be signed and updated yearly.
ALL forms must be signed and ready to turn in to the athletic office to obtain your Blue Card. You may also pick up the forms from the athletic office.

As well as the above forms, the athletic department needs a copy of the student's insurance card and up to date immunization record or copy of current exemption form.
